Pandora is struggling to block users outside of the United States because their licensing agreements do not reach so far. It is painful to watch. I do wish them luck.
But try as I might, I cannot think of how licensing could work in the absence of hard choices like this. When the (very general) rule that if one copies and redistributes content, one must have permission has lapsed (when cable was permitted to retransmit broadcasting without paying, for example) licensing has failed, and regulatory solutions have had to be adopted. Not good.
